Sorry, it seems like a basic question but I am going through the question types and can't quite find it.
I am just asking one question in my survey but want to also ask for name, email and age (or date of birth) after answering my question.

Email address: [ ]
Year of Birth: [ ]
It's just to be able to email later those who want to be informed on the topic they picked in my main question.
So as a result would like to have list of names and emails from those participants who answered in a certain way to my main question. Thanks.
