Yes, with html and css and maybe some Javascript you can do this on your own, "with a little bit of code". There are actually examples about this in the forum.

Provide your LS version and where it is installed (own server, uni/employer, SaaS hosting, etc.).
Always provide a LSS file (not LSQ or LSG).

At Limesurvey v3.x tooltips are kind of build-in. The new Limesurvey 3 themes are using Bootstrap which offers a tooltip feature.
