Is there a possibility that one person can fill out the same survey several times? Maybe with a login-code?
The questionnaire will be filled out together with children, and because they don't have e-mail addresses the survey invitation will be sent to their teacher.

Yes, that's possible. You can set the token option "Uses left" to number greater than 1. This will be the number of allowed answers for the token. Please check the documentation: www.limesurvey.org/manual/Tokens .
