Instead of many words just a screen shot from a different survey system. I haven't really found the right keywords to find examples or workarounds on the limesurvey website. To me this looks more like a feature request, but to be sure I just ask.
The others fields would be used in followup array questions as well.

No, I just basically implemented it as they described. Worked great.
