Array with text box for other was created by RaCMJS
Is there a workaround to create an array that includes a textbox for "other" that is IN the array list? For example:

How much do you love your...
[Not at all, A little, Somewhat, Very much]

Cat
Dog
Fish
Other pet: [TEXTBOX]
