In lime survey the registration forms and fields are autogenerated in user side. I need to add some static fields like

1) country drop down, on select drop down DO and ajax or J query call.

2) User photo upload field in registration form.

3) Payment gateway integrations to complete registrations.

Also below are the requirements to be implemented with lime survey.

Static fields should be added in the auto generating forms as well as DB Tables in Lime Survey.

Multiple(2 to 4 forms) registration forms to capture user data. (Similar to forms attached PFA)
