Error when upload file

Error when upload file was created by nurmansetyawan
Hi, I have an issue when uploading file from client PC (user).
The error message is
"/index.php/uploader/index/mode/upload/sid/656794/preview/0/fieldname/656794X1X2/"

but when I upload from server, its work.

I dont even know what should I do, cz i cant find a spesific solution form forum.
