Where should be of prioritizing sub-questions of 1 to 4. (See the values ​​of each sub-question within parentheses)

First question:

How I can do to determine that it can only enter values ​​from 1 to 4 within each sub-question?

Second question:

How I can do to determine that the number is not repeated in other subpreguntar?. Words can not be repeated in another sub-question numbers.

En donde se deben de priorizar las subpreguntas del 1 al 4. (Ver los valores de cada subpregunta dentro de los parentesis)

Primer pregunta:

Cómo puedo hacer para condicionar que solo se puedan ingresar los valores del 1 al 4 dentro de cada subpregunta?

Segunda pregunta:

Cómo puedo hacer para condicionar que el número no se repita en otra subpreguntar?. Es decir no pueden repetirse los números en otra subpregunta.
